Eptifibatide is a glycoprotein IIb/IIIa class platelet inhibitor drug used to reduce ischemic cardiac events in specific patient populations. A protein found in the venom of a southeastern pygmy rattlesnake is used to make eptifibatide. FDA has approved this medicine for the medical management of unstable angina (UA) and non-ST elevation myocardial infarction (NSTEMI). FDA also approved eptifibatide for patients undergoing PCI, including intracoronary stenting. The IMPACT-II trial proved that eptifibatide use with heparin and aspirin reduces ischemic events following a percutaneous coronary intervention (PCI), especially in individuals with unstable angina. This activity outlines the indications, mechanism of action, administration, adverse effects, contraindications, warnings, precautions, monitoring, and toxicity of eptifibatide.

Eptifibatide is cleared by both renal and non-renal mechanisms, and renal clearance accounts for about 40% of total body clearance [21]. Severe renal dysfunction prolongs the duration of action of eptifibatide, as has been shown in three patients [22]. One patient with acute renal insufficiency had prolonged inhibition of platelet aggregation; one with acute on chronic renal insufficiency had an intracerebral hemorrhage and normal platelet aggregation was restored by hemodialysis; in a third, with end-stage renal disease, platelet function returned to normal after hemodialysis.

The pharmacokinetics of eptifibatide during steady-state infusion have been evaluated in an open study in 31 patients with mild, moderate, or severe renal impairment [23]. There was a strong correlation between eptifibatide clearance and creatinine clearance, and the break point for the purpose of dosage adjustment was a creatinine clearance of 56 ml/minute. In patients with moderate or severe renal impairment, clearance rates and steady-state concentrations of eptifibatide were respectively about 50% lower and almost two-fold higher than in patients with normal renal function or mild renal impairment. Inhibition of platelet aggregation exceeded the clinically significant threshold of 80% in all the groups. Five subjects had a mild bleeding event and four had mild to moderate non-bleeding events; none of the events required intervention. The authors concluded that in patients with renal impairment infusion doses should be adjusted accordingly, but that bolus doses need no adjustment.
